With a LifeWise Passport plan, you have a choice of five options for your deductible and office visit copays. Choose in-network deductible from $500 to $5,000. Office visit copays are $20, $25 or $30, depending on the deductible option you choose. Insurance covers professional, naturopathic and chiropractic office visits, with copay. Preventive screenings, immunizations and health education are covered in full. Excellent coverage at an affordable price.

LifeWise HSA Plan

An Arizona Lifewise Health Savings Account (HSA ) is an account you fund to pay for your current and future medical expenses. Qualified expenses means health care necessary to prevent or treat a physical or mental disease or illness. Before establishing an HSA, you must choose a qualified high-deductible health plan like LifeWise HSA 20 Plans and meet certain requirements. Then you can use funds from your HSA account to pay for your current health expenses or save your funds.
