What is a PPO? PPO plans are designed for people who want to see licensed health care professionals without a referral.

Health Net Arizona PPO Advantages include: Visit any physician and be eligible for coverage. You need no referrals to see specialists. There are no claim forms when using in–network providers. You have lower copayments and coinsurance when visiting in–network providers.

What is an HMO? HMO plans are designed for people who want more predictable health care costs and who wish to use one doctor to coordinate all of their care needs.

Health Net Arizona HMO Advantages include: Set copayments for office visits, preventative care services, prescription coverage and no claim forms. Emergency care coverage is available worldwide.

What is a High Deductible (HSA–COMPATIBLE) PPO Plan? A high deductible PPO plan is designed to be used with a Health Savings Account (HSA).

Health Net Arizona HSA–Compatible Advantages include: Low monthly premiums, preventative care coverage from in–network providers exams, immunizations and screenings. You are allowed your choice of doctor with no referrals required. Roll your HSA dollars over from year to year. You pay the full cost of services and prescriptions until you meet the deductible. Then your plan coverage helps cover medical costs.

HealthNet Insurance Oregon

Health Net Oregon Diamond 15 – In–network doctor visits are $15 with no deductible. This plan also offers preventative benefits, an additional accident waiver and Well Net program benefits.

Health Net Oregon Pearl 25 HMO – Offers comprehensive care benefits, preventative care benefits and $25 copay. Included are Well Net benefits that allow for chiropractic, naturopathy, acupuncture and message therapy treatments. This plan is available to residents of Clackamas, Multnomah and Washington counties.

Health Net Oregon Topaz First Dollar – Offers a first–dollar benefit which means you get an immediate spending allowance for a number of services before you are required to meet your deductible. A variety of deductibles helps you manage your health care budget.

Health Net Oregon Emerald 40 – No deductible applies for office visits of preventative care. You pay just $40 dollars to see your in net–work doctor. An accidental injury deductible is also included in this plan.

Health Net Oregon Garnet 50% – This plan offers one of the lowest rates available. Choose a zero deductible plan for immediate benefit access. Choose a high deductible plan for after you pay for smaller medical bills. Either way, this plan covers benefits at 50% until you reach the out–of–pocket maximum safety net.Then it pays 100% for the rest of the year.

Dental and Vision Option Plan – Add dental and vision to your Health Net Plan. You can choose your own dentist with Health Net Dental. Use and in–network provider for vision and pay a small copay.

Quick Net Option – This short term medical coverage is designed for people who are between jobs, in life transition, no longer on parents plan or are traveling. It is available for daily (30–day minimum) or monthly coverage.

For small businesses they offer a wide selection of HMO, PPO and POS (point–of–service) plans to choose from along with their USelect plan, an innovative consumer–directed PPO that offers personalized health care options.
