Aetna Golden Medicare Plan (Arizona only)

Aetna Golden Medicare Plan is a Medicare Advantage HMO that gives you access to tens of thousands of network doctors nationwide. This allows you to get more benefits for less money than offered by Original Medicare plans for most services.

Other plan features include:

• Predictable out-of-pocket costs
• No need for referrals for covered services within their Open Access HMO plans in select areas
• Part D prescription drugs are all covered when you choose an “MAPD” plan
• $0 copays for preventive care, including routine physicals and annual screenings
• Fitness center benefits and a gym membership, at no extra cost to you
• Allowances for hearing aids and eyewear

You must be entitled to Medicare Part A and continue to pay your Part B premium and Part A, if applicable. Higher costs apply for out-of-network services. Precertification, is requested for certain services. Providers must be licensed and eligible to receive payment under the federal Medicare program. Coverage is provided through a Medicare Advantage organization with a Medicare contract. Plan benefits, limitations, service areas and premiums are subject to change on January 1 of each year.

Gives you all of the Essentials plan benefits listed above, plus:

• $0 deductible on brand-name prescription drugs
• Lower non preferred brand copays when you need a prescription drug not available in generic form

Aetna Medicare Rx Premier® Plan

• Covers all Medicare Part D brand-name and generic prescription drugs
• $0 deductible on brand–name and generic prescription drugs
• Unlimited generic prescription drug coverage in the gap
• Discounted pricing for brand–name prescriptions during the coverage gap (donut hole)
• Savings on three months of covered prescriptions using Aetna& mail–order program
• Predictable copays

A Medicare Advantage Private Fee–for–Service plan works differently than a Medicare supplement plan. Your doctor or hospital is not required to agree to accept the plan’’s terms and conditions, and thus may choose not to treat you, with the exception of emergencies.
