If you live in Franklin, Benton or Yakima Counties, you have the choice of enrolling the Columbia Community Care Enhanced Plan (HMO) or Plus Plan (HMO). Your Part D coverage for prescription drugs is included in all plans except the Basic Plan.

All Medicare Advantage Plans agree to stay in the program for a full year at a time. Each year, the plans decide whether to continue for another year. Even if a plan decides not to continue, you will not lose Medicare coverage.

Columbia Community Care Medicare Advantage Plans are HMO plans with a Medicare Advantage contract. Anyone who resides in the service area and who has Medicare Parts A and B may apply.
