Copy of the most recent filed Federal Form 1040C
for a husband-wife operation.
Partnership Agreement or most recent Federal Form1065 for a partnership

LifeWise Health Plan of Oregon groups of 2-5 employees require:

Most recent State Unemployment Tax Act (SUTA) filing including Form 132 for employees.
Partnership Agreement or a Federal Form 1065, if a partnership.

Form 132 Oregon State Wage & Tax Form.(if wage and tax form not available, contact us)
