    for a husband-wife operation.
  • Partnership Agreement or most recent Federal Form1065 for a partnership

LifeWise Health Plan of Oregon groups of 2-5 employees require:
  • Form 132 Oregon State Wage & Tax Form.
  • Business Employment Verification Statement by a CPA or CMA. Download now
LifeWise Health Plans
PacificSource groups of 2-3 employees (including sole-proprietors) require:
  • Most recent State Unemployment Tax Act (SUTA) filing including Form 132 for employees.
  • Partnership Agreement or a Federal Form 1065, if a partnership.

ODS Health Plans groups of 2-3 employees require:
  • Form 132 Oregon State Wage & Tax Form.
  • Copy of the most recent filed Federal Form 1040.
Kaiser Permanente Northwest groups of 5 or less employees require:
  • Form 132 Oregon State Wage & Tax Form.(if wage and tax form not available, contact us)
  • Signed census
