You are eligible for Providence Health Plans medical insurance if you live within their Oregon service area (Portland metropolitan, Willamette Valley, Hood River and Central Oregon), are under age 65, and not eligible for Medicare.
Individual & Family Plan Features
Find the plan that meets the health care needs of you or your family, as well as your budget. You can choose from three OR health insurance options: Optimum, Value or HSA. Each plan offers a variety of features and deductibles to meet your needs.
Optimum Plans
Optimum Plans offer you the most comprehensive coverage. There is no copay for preventive care services for your personal physician or health care provider office visits. There is a low cost $20 copay for other physician services. This copay is available before meeting the deductible. You aren't required to pay your deductible before you get the medical advice you need to healthy. You may also choose to see a non-participating provider. If you choose that option, you'll pay 40 percent coinsurance. There are five affordable Optimum Plans with several deductibles from which to choose.
Value Plans
Value Plans offer similar coverage as Optimum Plans. Preventive healthcare services are covered in full. Personal physician or healthcare provider office visits are available for a $30 copay, before deductible. The deductible is not waived for visits to specialists. When you reach your deductible, specialist visits are 30 percent coinsurance for participating providers or 50 percent coinsurance for non–participating providers. There are five Value Plans with several choices of deductibles.
Both Optimum and Value Plans Feature:
- Accidental injury benefit for all covered services before meeting deductible.
- Hospital, surgery, X-ray, laboratory services from participating physicians and facilities.
- Emergency care (In-Plan and Out-of-Plan) for a $250 copayment, before deductible.
- Prescription drugs benefits available through participating pharmacies for generic and brand-name drugs. No deductible!
- Travel Benefits and Nationwide coverage using their national network of participating providers.
Providence Health Care Health Savings Account (HSA)Plan
HSA Plans work together with a Health Savings Account and provide comprehensive coverage with a low monthly premium. This account allows you to set aside tax–deductible, interest–earning funds in a health savings account (HSA). Use these funds to help cover your deductible and other qualified medical expenses. Roll over unused funds each year and enjoy tax–sheltered investment and retirement planning options.
