Group Health Cooperative
offers four senior medical
Medicare Advantage HMO plan
options: Clear Care Vital (HMO),
Basic (HMO), Essential (HMO) & Optimal (HMO). They also offer two PPO plans:
Clear Care Prestige (PPO) and Elite (PPO)
and one Medicare Advantage HMO SNP
(Special Needs Plan). Clear Care
Sound (HMO) healthcare plan Group Health’s
Clear Care℠ plans
are coordinated care plans with a Medicare Advantage contract.
Members can choose their own doctor, and have access to two
fitness plans (SilverSneakers and EnhanceFitness),
well–adult exams, home health care, and virtually no
paperwork. In addition we offer an optional dental benefit, as
well as the Plus Package which includes vision, hearing, travel,
and alternative therapy benefits. Three prescription drug plans
are also available (one is available only in King and Pierce, WA
counties).
2010 Benefit Information
For the Clear Care Vital plan, Clear Care Basic plan, Clear Care
Essential plan, and Clear Care Optimal plans, coverage is
available in the following counties: Grays Harbor (partial),
Island, King, Kitsap, Lewis, Mason (partial), Pierce, San Juan,
Skagit, Snohomish, Spokane, Thurston, and Whatcom counties.
Clear Care Summary of Benefits (PDF)
Clear Care Prestige and Clear Care Elite is offered to
individuals that live in Benton, Franklin, Yakima, and part of
Clallam and Jefferson counties.

Clear Care Sound is offered to individuals that live in King
and Pierce counties and have been diagnosed with diabetes,
congestive heart failure, or coronary heart disease.
