Group Health Cooperative offers three senior medical Medicare Advantage HMO plan options: Clear Care Basic, Essential and Optima and one Medicare Advantage HMO SNP (Special Needs Plan) Clear Care Sound healthcare plan Group Health's Clear CareSM plans are coordinated care plans with a Medicare Advantage contract. Members can choose their own doctor, and have access to two fitness plans (SilverSneakers and EnhanceFitness), well-adult exams, home health care, and virtually no paperwork. In addition we offer an optional dental benefit, as well as the Plus Package which includes vision, hearing, travel, and alternative therapy benefits. Three prescription drug plans are also available (one is available only in King and Pierce, WA counties). 2009 Benefit Information Clear Care Sound is offered to individuals that live in King and Pierce
counties and have been diagnosed with diabetes, congestive heart failure,
or coronary heart disease. |
