As a Medicare beneficiary, you have a choice of different Medicare options. One option is the Original (fee–for–service) Medicare Plan. Another option is a Medicare health plan, like CIGNA Medicare Select Plus Rx. You may have other options too. You can make the choice that is best for you; you are still in the Medicare program.

You can join the CIGNA Medicare Select Plus Rx plan if you are entitled to Medicare Part A and enrolled in Medicare Part B and live in the plan service area (Maricopa County and City of Apache Junction. Individuals with End Stage Renal Disease are generally not eligible to enroll in the CIGNA Medicare Select Plus Rx plan unless they are an Cigna member and have been insured by them since their dialysis began.

CIGNA Medicare Select Plus Rx Chronic Care

You can join the CIGNA Medicare Select Plus Rx–Chronic Care plan if you are entitled to Medicare Part A and enrolled in Medicare Part B and live in the service area (Maricopa County and City of Apache Junction). You must have been diagnosed by your doctor with (high cholesterol), Diabetes, Hypertension or Chronic Heart Failure (CHF). Please call CIGNA Medicare Select Plus Rx–Chronic Care to see if you are eligible to join the plan.

For certain covered services, the maximum amount that you will be required to pay during a calendar year is $3,000. Should you reach this maximum, no further copayment/coinsurance would be required for those covered services for the remainder of the calendar year.

The CIGNA Medicare Access Plus Rx Plan gives you the freedom to see any health care provider that accepts Medicare payment and CIGNA’s terms and conditions of payment. Before receiving services from any provider you need to make the provider aware you have CIGNA Medicare Access Plus Rx Plan coverage in place of Originalb Medicare. IT is important that you show your CIGNA Medicare Access Plus Rx Plan ID card every time you visit a healthcare provider.

The CIGNA Medicare Access Plan gives you the freedom to see any health care provider that accepts Medicare payment and CIGNA’s terms and conditions of payment. Before receiving services from any provider, it is important that the provider is aware you have CIGNA Medicare Access Plan coverage in place of Original Medicare. A Medicare Advantage Private Fee-for-Service plan works differently than a Medicare supplement plan. Your doctor or hospital is not required to agree to accept the plan’s terms and conditions, and thus may choose not to treat you, with the exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may choose not to provide health care services to you, except in emergencies.
